The Glade:

After going through the portal you will appear here:

Kill the Sakkara Demon that is guarding the gate:

When it dies it will drop a Dimension Gate Glyph:

Take the Glyph and walk back into the Gate (into the mouth of the skull), this will destroy the Gate:
