Item Modifiers

First there is a large list of modifiers that can appear on ANY item:

Modifier Effect
Banish Undead: +X This works like the Undead Death potion and keeps the undead from rising multiple times before they finally die for good.
Chance for open wounds: +X% This mod will increase the chance that an enemy will receive wounds when you hit them. Wounds cause damage over time and the higher the damage of the hit that caused the wound the higher the damage suffered from the wounds.
Chance of finding special items: +X% This modifier increases the chances of finding magic, rare, set or unique items instead of normal items.
Chance to gain gold by hitting opponent: X% Increases the chance that gold will drop from monsters each time they are hit.
Chance to land a critical hit: X% A critical hit is a hit that does 2x, 3x, or 4x the regular damage. This mod increases your chances to do so.
Chance to trap flying opponents: +X% Allows trapping of flying creatures. When hit trapped creatures are literally held to the ground by green tendrils. The higher the percentage the greater your chance of trapping creatures.
Damage received reduces gold, saving health: X% The percentage is how much damage is taken from gold instead of life. EXAMPLE: The mod is 10%, your life is 100, your gold is 100. If you get hit for 50 damage, then 10% of that will be taken in gold so you end up with 55 Life and 95 Gold.
Each hit drains own life energy: X% Opposite of the one above, this mod decreases your own life. The percentage on the mod is the percent of the damage you inflict that will be removed from the health of your character.
Each hit draws life from opponent: X% This mod allows you to leech life from monsters. The percentage on the mod is the percent of the damage you inflict that will be returned as health to your character.
Enables the attacking of animals: Allows you to attack animals such as rabbits, birds, deer, etc.
Enemies die on sight: X% The percentage on this mod identifies how often an enemy will die on sight.
Enemies die on sight below level: X+X Enemies a certain level below you will drop dead as soon as they come into view. Note that they won't give you any experience and they very rarely drop an item more than the weapon they are using, which is usually junk. The first number on the mod is the number of levels below you that an enemy must be to die on sight. I am not sure what the second number means but some think that it refers to the radius around you that the enemies must be in to count as being "on sight".
Experience gained from monsters: +X% Increases the amount of experience you gain from enemies. EXAMPLE: If the mod is +10% and you normally get 100 exp. from a monster. You will now get 110 exp. from that monster.
Extended Day: +X% Increases the length of day time.
Extended Night: +X% Increases the length of night time.
Far Sight: +X Increases the distance from your location in which you can see monsters on the mini-map.
Find Potion: X% Increases the chance that monsters will drop potions when they die.
For attack and defense: +X Increases your defense and attack stats by the given amount.
Increased light radius: +X Increases your light radius enabling you to see further at night or in dark caves.
Percent to all resistances: +X% Raises all your resistance stats by the given percentage.
Regeneration Special Move: +X% Reduces the amount of time that special moves (non-spell combat art) take to regenerate, allowing you to perform them again sooner.
Regeneration Spells: +X% Reduces the amount of time that spells take to regenerate, allowing you to cast them again sooner.
Relationship between your health and damage taken: x% The lower your health is, the less damage you will take, based on the percentage.
Resist Stun: +X Reduces the chances that an enemy will successfully stun you. (Stun is what a Cave Fish does when hitting you that prevents you from moving or attacking for a short period of time.)
Shock Opponent: +X It seems that it both stuns and damages an opponent.
Spell Resistance: +X This reduces the chance of being effected by spells cast at you by an opponent.
Split: X% Splitting is when an enemy clones itself when it is hit, this increases that chances of it doing so.
To all combat arts: +X Increases all your combat arts by the given number.
To all magic spells: +X Increases all your spells by the given number.
To all skills: +X Increases all your skill stats by the given number.
Wounds increase damage dealt: +X% This mod adds additional damage to monsters per hit, based upon your own health. Additional damage  =  (x/100)(max health  -  current health). If you have full health no additional damage will be dealt. So the lower your health, the greater the additional damage will be.
* Some item mods will say "vs." a certain monster type or "to" a certain monster type... the mods are the same as described above but only come into effect when fighting that monster type.

Next there a number of special modifiers that ONLY appear as certain set item bonuses... I may have skipped one or two that I thought were self-explanatory so let me know if I was wrong and any should be added to this list. (Big thanks to DaveO for letting me use a bunch of his screenshots and thanks to anyone who donated their screenshots to him!):
