This quest starts with a magician who is inside the tavern by the Eastern road out of town:

The quest flag will direct you to the following location but there are Cave Fish all over the area so feel free to kill them off while you do other quests:

Each Cave Fish you kill will drop an Intact Lionfish Spleen when it dies:

When you have 15 Spleens bring them back to the Magician for your reward:
