NOTE: If you DID NOT chose to have Vilya join your party back in the Mines the message "From now on Vilya will accompany you again" will pop up on the screen about halfway to the above location and she will appear in your party.

There you will find Ceriri (be careful of the poison spikes he casts):

When he dies he will drop a Dimension Gate Glyph:

Take the Glyph and enter the portal:
