Draconis Mortis:

There are two ways to reach Draconis Mortis...

The first way (that is the one the game intends you to take) is in Crow's Rock Castle. Either take the Timberton portal or the Icecreek Pass portal and walk to Crow's Rock. Once there go to the Northern part of town, you are looking for a path just past the horse dealer:

The path leads a short distance North to this location:

Enter the cave:

Go to the last room where you see the portal icon, there you will find a closed portal with a magician guarding it:

Kill the magician:

When he dies the portal will open:

Go through it and you will be at the island where the dragon is:

 The second (and I think easier) way is to take the Mascarell Main Portal, then walk Northeast to this location:

To reach the small island that Draconis Mortis is on use either a Combat Jump or Phase Shift to jump from the shore to the small area next to the cliff, then from there to the island, the path you need to jump is marked with yellow dots above. Draconis Mortis is marked by the yellow X.
