This quest starts with a medic who is standing outside of the Southern most building inside town:

You need to go a fair distance North to a small town (which is the Easter Egg Tristram Town), his is the house that is separate from the rest of the town and it has a witch standing in the yard:

Just inside his house you will see the Headache Remedy on the ground:

Bring it back to Lothar for your reward:
